About this opportunity
The Care Management Program establishes Direct Service Purchase (DSP) Provider Enrollment Agreements throughout the fiscal year to purchase in-home and community services on a per-unit basis according to individual care plans. DSP-issued agreements reimburse providers for services such as Community Living Supports (In-Home and Residential), Personal Care, Homemaker, In-Home Respite, Out-of-Home Respite, Chore, Home Delivered Meals, PERS, Counseling, Financial Management, Nursing Services, Private Duty Nursing, Medication Management, and Non-Emergency Medical Transportation.
